Dr. Ghanshyam Gupta performs procedures conveniently located in the same building as the office at the Advanced Surgery Center (Suite 100). He also performs them at Shady Grove Adventist Outpatient Surgery Department approximately 1 mile from the office. Call to schedule a consult today.
Preparing for your GI health visit:
- Please arrive 15 minutes prior to your appointment time to fill out the appropriate paperwork.
- We will make a copy of a valid photo ID and insurance card.
- Please be sure to bring a written list of your current medications.
- Please bring with you copies of your recent test results such as blood work, X rays and CT scans and surgery or procedure reports as well as any emergency room visit summaries.
It is the patient’s responsibility to know the requirements of their insurance. If your insurance plan requires a referral, please bring the referral to your appointment and give it to our receptionist when you arrive and be prepared to pay any co-payments at that time. If you are unsure as to whether or not you need a referral or have a co-payment, please call the member services number on the back of your insurance card. Co-payments and balances are collected at the time of service. We accept cash or VISA, Mastercard, American Express and Discover. Personal checks are accepted from existing patients.
